Welcome to Health Coaching Services at B.T. Wellness LIVE with Tracey Walter, INHC! This informed consent and waiver document contains important information about health coaching services and related business policies. Please note that we are a fully insured entity.

 

Health Coaching Services

Health coaching is a general term for the process of facilitating healthy and sustainable behavior change and transforming individual goals into action. Health and coaching include exploration of one's sleep routine, exploration of food and lifestyle choices, exercise, and hormones with an overall focus and journey towards healthy living.

Risks and Benefits

Health coaching can have benefits and risks. Since health coaching involves various life change goals it's important to understand what health coaching is and is not.

The role of Tracey is not to prescribe or assess micro- and macronutrient levels, provide health care, medical services or to diagnose, treat or cure and specific disease, condition or other physical or mental ailment of the human body. Rather, to mentor, guide who has been trained in holistic health coaching to help clients reach their own health goals by helping clients devise and implement positive, sustainable lifestyle changes.

Tracey Walter, INHC is not acting in the capacity of a medical doctor, licensed dietician-nutritionist, or personal trainer and any advice given by Tracey Walter INHC is not meant to replace the advice of these professionals. If you are under the care  of a health care professional or currently use prescription medication(s), you should discuss any dietary changes or potential dietary supplement use with your health care provider and should not discontinue any prescription medications without first consulting your health care professional. The AO biometric full body scan by Solex can be used to assist your doctor in determining root cause but does not replace the need of examination, diagnosis or treatment.

Cancellation Policy and Contact

Once a free or paid health coaching session is scheduled you will be expected to keep the appointment and pay unless a 24-hour notice is provided. If possible, we will work to reschedule the session. We understand that unplanned emergencies arise that can prevent you from making your session, however we ask that you communicate with us in a timely manner so we can offer the time to someone else in need. 

The best way to reach Tracey is by email and she or someone in her office will return the message soon as possible, usually within 24 hours. If you face an emergency, please contact 911 or go to the nearest emergency room.

Confidentiality and Professional Records

In general, the privacy of all communications between Tracey Walter, INHC and client will remain confidential. In the case where the client may be in danger, is being abused or may harm himself or someone else, Tracey is obligated by the law to report such.

Professional Fees and Billing

Payment for services is an important part of your work with B.T. Wellness LIVE. Payment is expected at the time of service and payment plans are available if needed. In the case of a payment plan, a card will need to be left on file to bill on the 1st or 15th of the agreed upon payment. In the case of a missed payment past 60 days, a case will be opened in small claims court.

Health Coaching Services Release/Waiver

I have chosen to work with Tracey Walter, INHC and understand that the information received should not be seen as medical, nursing or counseling advice and is not meant to take the place of seeing a licensed health professional. I acknowledge and take full responsibility for my life and well-being, as well as the lives and well-being of my family and children (when applicable) and all decisions made during and after these services. I understand that no promises have been made to me as to the results of these health coaching services.
